APPLICATION FOR POSTDOCTORAL FELLOWSHIP


The application form consists of a face sheet and an abstract and budget page, which should be filled out completely and submitted with the 3 Appendices described below. Please see the foundation's website Guidelines (www.lalorfound.org/guidelines) for eligibility details.

SPECIFICATIONS CONCERNING APPLICATIONS

1. The proposed fellow is responsible to see that the signed, completed application and all supporting information is RECEIVED by the foundation not later than 5:00 p.m. on January 15, or on the next working day, should the deadline fall on a weekend.

2. The application and all material submitted should be typewritten on 8 1/2" x 11" or A4 paper. Please do not change the font size or margins of the forms provided. The narrative portion of the application should have 1” margins all the way around, except that applicants using A4 paper should leave 1.5” at the bottom of each page. The prospective fellow’s name should appear in the upper right hand corner of each page. Forms are available online, but proposals should be submitted in hard copy.

3. One original and 4 copies of the application and accompanying documents should be addressed to the Fellowship Manager, The Lalor Foundation, Inc., c/o GMA Foundations, 77 Summer Street, 8th Floor, Boston, MA 02110-1006 USA. Please print double sided whenever possible. Applications should be held by clips or rubber bands, no staples please; folders, report covers, or other types of binders do not fit files and are discarded. Feel free to contact fellowshipmanager @ gmafoundations.com with any questions not answered in these instructions.

4. Please fill out the face sheet and abstract and budget page. You may type directly into the form, but please do not change the font or margins.


APPENDICES
The application is to be accompanied by three appendices as follows:

Appendix A – Project Narrative (5 to 7 pages only)

1. The plan of work for the project should concisely describe the present state of knowledge regarding the problem, the scope, specific objectives and importance of the work which will be accomplished during the period.

2. Please outline in what fashion this research can potentially be applied to the regulation of human fertility or to a method of birth control. This information may be helpful in making a choice between applications which are equally attractive from a purely scientific point of view.

Appendix B – Letters of Support

1. Independent Expert Reviews: The applicant should contact two independent experts who will review the proposal, comment on its scientific merit, and give their opinion of the ability of the lab to supervise and support the project. These reviewers should be experts in the field who do not know the applicant personally and should state their own qualifications to perform the review. Representatives of any institutional unit that stands to receive financial benefit from a fellowship are not considered to be independent reviewers. Preference is for reviewers from outside the applicant’s institution. The instructions and form included in the application material should be sent to the expert reviewer by the applicant.

2. Reference: The applicant should also submit one letter of reference from someone who does know the applicant’s work and who can comment on the applicant’s abilities as a researcher. This should not be the person who will be the mentor/sponsor on the project but rather someone like a dissertation advisor or committee member. The instructions and form included in the application material should be sent to the reference by the applicant.

3. The applicant should request a letter from the proposed sponsor/mentor outlining in layman’s terms the relevance/importance of the project to the foundation’s focus; the training/experiences during the project that will assist the fellow to apply successfully for continuing funding and to progress toward independence; the ability of the fellow to carry out the proposed project. The sponsor/mentor should prepare the letter of support utilizing the instructions included in the application material.

All letters of support should be returned to the applicant for inclusion in the proposal packet. These letters should be on the forms provided in these guidelines and in sealed envelopes, marked “Confidential,” and signed by the reviewer on the envelope flap. All letters of support should include the NIH Biographical Sketch of the person writing the review. Proposals are not complete without these letters.

APPENDIX C - Institutional Tax Status and Approvals

The Appendix is to set forth information concerning the institution’s eligibility to receive grants under U.S. tax laws and approvals of the project by reviewing bodies.

1. Attach a copy of the institution’s IRS 501(c)(3) determination letter. Institutions outside the United States should attach their country’s equivalent nonprofit designation.

2. Please complete the Institutional Representation included with the application forms.

OBLIGATIONS OF THE GRANTEE

The applicant institution, if awarded a grant, agrees to the following:

1. To maintain funds awarded by the Foundation in a separate fund (which may be a separate bank account or a separate bookkeeping account) maintained for charitable purposes.

2. To maintain records of the receipts and expenditures and to make its books and records available to The Lalor Foundation at reasonable times.

3. Not to use any of the funds granted –
a. to carry on propaganda, or otherwise to attempt, to influence legislation (within the meaning of section 4945(d)(1) of the U.S. Internal Revenue Code);
b. To influence the outcome of any specific public election, or to carry on, directly or indirectly, and voter registration drive (within the meaning of section 4945d(d)(2) of the U.S. Internal Revenue Code);
c. To make any grant which does not comply with the requirements of section 4945(d)(3) or (4) of the U.S. Internal Revenue Code; or
d. To undertake any activity for any purpose other than one that is charitable, scientific or educational.

4. To submit full and complete narrative and financial reports as scheduled and described in the award letter.

5. To return any portion of the amount granted which is not used for the purposes of the grant.
